Compliant CTP Documentation

CTP reimbursement begins with good documentation that takes into account payor requirements. Since there are hundreds of third-party payors in the United States, this alignment can often be a challenge for clinicians and physicians who may want to apply CTPs to their wound healing practice. This white paper provides some guidance on documentation practices that support reimbursement of CTPs. Discussion includes documentation of why a CTP is “reasonable and necessary,” medical necessity, underlying contributing medical diagnoses, ulcer stage, patient medication list, and other essential components.
